Follicular dendritic cell sarcoma and its response to immune checkpoint inhibitors nivolumab and ipilimumab
Follicular dendritic cell sarcoma (FDCS) is a rare and unusual cancer that arises from sustentacular cells of the lymph node that present antigen to B cells, rather than lymphocytes themselves. While surgery for primary disease is still paramount in primary management, for unresectable, recurrent and metastatic tumours, FDCS is frequently treated with anthracycline-based lymphoma chemotherapy regimens. In recent years, it is clear that Programmed Cell Death 1 (PD1)-directed immune checkpoint inhibitors...

Incidental congenitally corrected transposition of the great arteries (ccTGA) in an adult with suspected coronary artery disease: review on radiological features and pathophysiology
We report a case of a 46-year-old woman who has presented to a peripheral hospital with progressive exertional dyspnoea and chest discomfort. The resting ECG showed features of left-sided ventricular hypertrophy. The initial chest radiograph was reported as cardiomegaly. Initial echocardiography revealed left atrial dilatation and ‘left ventricular’ hypertrophy with normal ejection fraction. She was treated as possible coronary artery disease and was subsequently referred to our centre for CT coronary...

Cambridge experience in spontaneous bone regeneration after traumatic segmental bone defect: a case series and review of literature
High-energy traumatic long bone defects are some of the most challenging to reconstruct. Although cases of spontaneous bone regeneration in these defects have been reported, we are aware of no management guidelines or recommendations for when spontaneous bone regeneration should be considered a viable management option. We aim to identify how certain patient characteristics and surgical factors may help predict spontaneous bone regeneration. A total of 26 cases with traumatic segmental defects were...

Malignant peripheral nerve sheath tumour of thyroid: a diagnostic dilemma
Malignant peripheral nerve sheath tumours of thyroid are rare entities that can present a diagnostic dilemma. We present the case of a patient who presented with neck mass with a history of multiple neck surgeries and airway compression. The patient’s previous histopathology was mistaken for Riedel’s thyroiditis in an outside hospital, which delayed appropriate treatment leading to suffering on part of the patient and frustration on part of the physician. We emphasise that rare malignancies should...

Stereotactic body radiation therapy (SBRT) in the management of pulmonary spindle cell carcinoma
A 69-year-old woman underwent routine screening with CT scan of the chest, which showed a new right upper lobe lesion. Interval increase in size of the right upper lobe nodule over 3 months, prompted a CT-guided biopsy of the lung that confirmed a diagnosis of malignant pulmonary spindle cell carcinoma (PSCC) with 90% programmed death ligand 1 expression. Positron emission tomography CT demonstrated localised stage IIA disease. Given histologically proven PSCC and the rapid growth of her tumour,...

Supraclavicular left neck mass: an unusual presentation of metastatic testicular cancer in two older men
Two male patients, aged 55 and 75 years, were referred to the ENT and haematology team respectively, with an asymptomatic left supraclavicular neck mass. Investigations revealed metastatic primary testicular seminoma. CT contrast study of the thorax, abdomen and pelvis demonstrated an additional left para-aortic nodal mass in both cases. The initial presentation of a solitary left neck lump from a metastatic testicular seminoma is extremely unusual, especially in an older age group. The pattern of...

Awake and aware with ongoing ventricular fibrillation during LVAD treatment: is it possible?
Left ventricular assist devices (LVADs) are currently used as destination therapy or bridge to heart transplantation in patients with advanced chronic heart failure (CHF). It has been proved to reduce mortality and symptoms in these patients. Patients with advanced CHF are known to have increased risk of ventricular arrhythmias (ventricular tachycardia or ventricular fibrillation (VF)) despite the presence of LVAD. We report the case of patients with ongoing VF during LVAD treatment while being awake...

ADER Methods for Hyperbolic Equations with a Time-Reconstruction Solver for the Generalized Riemann Problem: the Scalar Case
Abstract The ADER approach to solve hyperbolic equations to very high order of accuracy has seen explosive developments in the last few years, including both methodological aspects as well as very ambitious applications. In spite of methodological progress, the issues of efficiency and ease of implementation of the solution of the associated generalized Riemann problem (GRP) remain the centre of attention in the ADER approach. In the original formulation of ADER schemes, the...

An Approximate Riemann Solver for Advection–Diffusion Based on the Generalized Riemann Problem
Abstract We construct an approximate Riemann solver for scalar advection–diffusion equations with piecewise polynomial initial data. The objective is to handle advection and diffusion simultaneously to reduce the inherent numerical diffusion produced by the usual advection flux calculations. The approximate solution is based on the weak formulation of the Riemann problem and is solved within a space–time discontinuous Galerkin approach with two subregions. The novel generalized...

A Third-Order Accurate Wave Propagation Algorithm for Hyperbolic Partial Differential Equations
Abstract We extend LeVeque’s wave propagation algorithm, a widely used finite volume method for hyperbolic partial differential equations, to a third-order accurate method. The resulting scheme shares main properties with the original method, i.e., it is based on a wave decomposition at grid cell interfaces, it can be used to approximate hyperbolic problems in divergence form as well as in quasilinear form and limiting is introduced in the form of a wave limiter.

A Multi-physics Methodology for Four States of Matter
Abstract We propose a numerical methodology for the simultaneous numerical simulation of four states of matter: gas, liquid, elastoplastic solids, and plasma. The distinct, interacting physical processes are described by a combination of compressible, inert, and reactive forms of the Euler equations, multi-phase equations, elastoplastic equations, and resistive MHD equations. Combinations of systems of equations are usually solved by coupling finite element for solid modelling...

The Notion of Conservation for Residual Distribution Schemes (or Fluctuation Splitting Schemes), with Some Applications
Abstract In this paper, we discuss the notion of discrete conservation for hyperbolic conservation laws. We introduce what we call fluctuation splitting schemes (or residual distribution, also RDS) and show through several examples how these schemes lead to new developments. In particular, we show that most, if not all, known schemes can be rephrased in flux form and also how to satisfy additional conservation laws. This review paper is built on Abgrall et al. (Computers and...

Superconvergence and the Numerical Flux: a Study Using the Upwind-Biased Flux in Discontinuous Galerkin Methods
Abstract One of the beneficial properties of the discontinuous Galerkin method is the accurate wave propagation properties. That is, the semi-discrete error has dissipation errors of order \(2k+1\) (\(\le Ch^{2k+1}\)) and order \(2k+2\) for dispersion (\(\le Ch^{2k+2}\)). Previous studies have concentrated on the order of accuracy, and neglected the important role that the error constant, C,  plays in these estimates. In this article, we show the important role of the error constant...

Weak Solutions for Navier–Stokes Equations with Initial Data in Weighted $$L^2$$L2 Spaces
Abstract We show the existence of global weak solutions to the three dimensional Navier–Stokes equations with initial velocity in the weighted spaces \(L^2_{w_\gamma }\), where \(w_\gamma (x)=(1+\vert x\vert )^{-\gamma }\) and \(0<\gamma \leqq 2\), using new energy controls. As an application we give a new proof of the existence of global weak discretely self-similar solutions to the three dimensional Navier–Stokes equations for discretely self-similar initial velocities which...

NK1R antagonist decreases inflammation and metastasis of breast carcinoma cells metastasized to liver but not to brain; phenotype-dependent therapeutic and toxic consequences
Abstract Substance P a neuro-immune mediator acts on Neurokinin-1 and -2 receptors (NK1R and NK2R). Inhibitors of NK1R are considered to be safe and effective approaches for cancer treatment since Aprepitant, a non-peptide antagonist of NK1R is widely used for chemotherapy-induced emesis and has cytotoxic and antitumor effects in various models for cancer. On the other hand, our previous findings demonstrated that systemic inhibition of NK1R may decrease cytotoxic anti-tumoral...

Advances and perspectives of dendritic cell-based active immunotherapies in follicular lymphoma
Abstract Follicular lymphoma (FL) is a remarkably immune-responsive malignancy, which is still considered incurable. As, standard immunochemotherapy is complex, toxic and not curative, improvement in FL care is now a crucial topic in hemato-oncology. Recently, we and others have shown that dendritic cell (DC)-based therapies allow a specific immune response associated with sustained lymphoma regression in a proportion of low-tumor burden FL patients. Importantly, the rate of objective...

'Never forget: fictionalising the Holocaust survivor with dementia
This article asks what the reasons are for the frequent linking of the image of the Holocaust with that of dementia in contemporary discursive and representational practice. In doing so, it analyses some of the numerous 21st-century examples of fiction, drama and film in which the figure of a Holocaust survivor living with dementia takes centre stage. It explores the contradictory cultural effects that arise from making such a connection, in contexts that include expressions of fear at the spectacle...

Dissociation between reduced pain and arterial blood pressure following epidural spinal cord stimulation in patients with chronic pain: A retrospective study
Abstract Purpose Acute pain and resting arterial blood pressure (BP) are positively correlated in patients with chronic pain. However, it remains unclear whether treatment for chronic pain reduces BP. Therefore, in a retrospective study design, we tested the hypothesis that implantation of an epidural spinal cord stimulator (SCS) device to treat chronic pain would significantly reduce clinic pain ratings and BP and that these reductions...
