Hi, I am Peter Gloviczki from Mayo Clinic, Editor-in-Chief of the Journal of Vascular Surgery: Venous and Lymphatic Disorders. We have several excellent articles in our September issue and I am pleased to introduce the four best papers that we selected for you.
We aimed to compare mechanochemical ablation (MOCA) and thermal ablation (radiofrequency ablation and endovenous laser therapy) for venous ulcer healing in patients with clinical class 6 chronic venous insufficiency.
Venous ablation (VA) is the recommended treatment of superficial venous insufficiency affecting the lower extremities. The safety and efficacy of the procedure in octogenarians have not been well studied. We postulate that VA in octogenarians is as safe and effective as in younger age groups.
Drs Son, O'Donnell, and colleagues from Tufts Medical Center in Boston have given us a timely report on the crucial role of the “combined” condition of chronic venous and lymphatic insufficiency, defined as phlebolymphedema, and its impact on venous leg ulcers.1 Based on a unique, mutually interdependent venous-lymphatic system, which has an inseparable outflow system, its physiologic drainage functions are interdependent.

Endovenous ablation is the gold standard for treatment of chronic venous insufficiency secondary to axial venous incompetence. The two most common ablation modalities, laser (endovascular laser ablation [EVLA]) and radiofrequency ablation (RFA) have been shown to be efficacious. This study found that all endovenous interventions were effective in improving the clinical severity of varicose veins at 1 year despite differing rates of persistent vein occlusion.
Pulmonary embolism (PE) remains a leading cause of death in pregnant women in the Western world. The use of a pregnancy-adapted algorithm in pregnant women with suspected PE was successful and safely avoided the ionizing radiation exposure from computed tomographic pulmonary angiography (CTA) in 32% to 65% of pregnant patients in their third to first trimesters, respectively.
