Split zygomaticus major muscle sling reconstruction for significant lower lip defects
Defects to the lower lip remain surgically challenging to reconstruct. Restoration of aesthetics and function are crucial, of which the aims include restoring the tissues of the lower lip, maintaining oral competence, speech articulation, and restoring lip height and the vermillion. Further surgical techniques continue to be explored, with local flap surgery being the generally preferred method. However, many reconstructions leave the patient with pronounced deformity and morbidity. In this case...

Letter to the Editor – The future of OMFS lies in creating pathways to implement the PMETB recommendations and inspiring our trainees. Enough time has been spent debating, we need to deliver!
The British Association of Oral and Maxillofacial Surgeons (BAOMS) has been at the centre of the transition of our specialty in the UK from a branch of dentistry to one of the 10 UK surgical specialties. In this role it has, at different times, pushed boundaries against resistance from other specialties, and redirected the ambitions of the deputy chair of the Postgraduate Medical Education and Training Board (PMETB) review to produce recommendations that were exactly what OMFS needed. The editorial...

UK Oral and Maxillofacial Surgery trainees join the specialist list at a similar age to other surgical specialists
It is 11 years since Cameron and Westcott published ‘Maxillofacial training is no longer than other surgical specialties’1. This showed that OMFS trainees completed training at ages comparable to their surgical peers. Much has changed in surgical training since then so an updated review was undertaken. Based on published training pathways specialty training in most surgical specialties should be ten years (two years foundation, two years core and six years specialty training). For OMFS specialty...

We need to drive change for the future and support junior trainees whilst maintaining the highest training standards
The oral and maxillofacial surgery (OMFS) community in the UK has always felt distinguished to be the only surgical specialty requiring dual qualification. There is no doubt that OMFS recruitment in the UK is in crisis, and we believe that the time has arrived to review the long training pathway. Policy-makers should think of alternative options to make the training programme more sustainable whilst maintaining the highest standards. The problem is serious, and the onus is on all consultants and...

Determinants of lost theatre capacity
Secondary care Trusts nationwide are continuing to fail the 18-week referral to treatment (RTT) target despite several initiatives to improve theatre efficiency (2018 NHS England review). A limitation of wasted theatre productivity is required to alleviate pressures on waiting lists. Productivity, which is a measure of treatment time as a proportion of available/allocated time, takes into consideration variations in operator performance, early (non-funded) theatre starts, and over-run, and its analysis...

Management of third molar surgery in HIV‐positive patients
Abstract Background Third molar surgery is a common procedure performed by oral and maxillofacial surgeons. This kind of surgery is predictable, and complications are infrequent. Immune deficiency is one of the considerations for the prevention of complications. HIV patients may be immune deficient. Third molar surgical procedures are associated with bleeding and increased risk of infection. Improvement in oral hygiene must be encouraged, such as pre‐operative scaling. Prophylactic antibiotics...

Human immunodeficiency virus interaction with oral and genital mucosal epithelia may lead to epithelial–mesenchymal transition and sequestration of virions in the endosomal compartments
Abstract Oral and genital mucosal epithelia are multistratified epithelial barriers with well‐developed tight and adherens junctions. These barriers serve as the first line of defense against many pathogens, including human immunodeficiency virus (HIV). HIV interaction with the surface of mucosal epithelial cells, however, may activate transforming growth factor‐beta (TGF‐β) and mitogen‐activated protein kinase signaling pathways. When activated, these pathways may lead to the disruption of epithelial...

HIV testing in the dental setting: A global perspective of feasibility and acceptability
Abstract HIV is no longer a death sentence with early diagnosis and effective treatment contributing to a good prognosis. Surveillance data, however, suggest that one in four people globally living with HIV are unaware of their HIV infection and remains at risk of transmitting their infection if having unprotected sex. A wider array of testing opportunities and settings are thus needed to address this issue of public health significance. Evidence suggests there is a role for the dental team, particularly...

Innate immune mechanisms to oral pathogens in oral mucosa of HIV‐infected individuals
Abstract A crucial aspect of mucosal HIV transmission is the interaction between HIV, the local environmental milieu and immune cells. The oral mucosa comprises many host cell types including epithelial cells, CD4 + T cells, dendritic cells and monocytes/macrophages, as well as a diverse microbiome predominantly comprising bacterial species. While the oral epithelium is one of the first sites exposed to HIV through oral‐genital contact and nursing infants, it is largely thought to be resistant to...
