Gastric polyposis and desmoid tumours as a new familial adenomatous polyposis clinical variant associated with APC mutation at the extreme 3'-end
Germline mutations of the APC gene, which encodes a multidomain protein of 2843 amino acid residues, cause familial adenomatous polyposis (FAP). Three FAP clinical variants are correlated with the location of APC mutations: (1) classic FAP with profuse polyposis (>1000 adenomas), associated with mutations from codon 1250 to 1424; (2) attenuated FAP (<100 adenomas), associated with mutations at APC extremities (before codon 157 and after codon 1595); (3) classic FAP with intermediate colonic...

Consequences of ED closures
Knowles et al in a paper published in EMJ have sought to examine the effect of emergency department (ED) closures on patient outcomes1. This is both welcome and timely, given the current debate on the viability of a number of EDs. Of necessity this is an opportunistic observational study as  the ‘intervention’ population are persons within the catchment area of five EDs in England closed in the period 2009 to 2011. The outcome measure was mortality. As part of the rationale for the paper the authors...

The impact of closing emergency departments on mortality in emergencies: an observational study
BackgroundIn England the demand for emergency care is increasing, while there is also a staffing shortage. This has implications for quality of care and patient safety. One solution may be to concentrate resources on fewer sites by closing or downgrading emergency departments (EDs). Our aim was to quantify the impact of such reorganisation on population mortality.MethodsWe undertook a controlled interrupted time series analysis to detect the impact of closing or downgrading five EDs, which occurred...

Pediatric hereditary angioedema: what the otolaryngologist should know
Purpose of review To review pediatric hereditary angioedema for otolaryngologists, with emphasis on articles within the past 12–18 months. Recent findings Biologic therapies are accepted for adult hereditary angioedema (HAE), but have been studied less for pediatric HAE. Recent literature supports expanded use of biologic agents in pediatrics as acute treatment and prophylaxis. Available agents include plasma-derived C1 esterase inhibitors (C1-INH) (Berinert, Haegarda, Cinryze), recombinant...

Cochlear MRI Signal Change Following Vestibular Schwannoma Resection Depends on Surgical Approach
Objective: Information on cochlear MRI signal change following vestibular schwannoma (VS) surgery by the retrolabyrinthine approach (RLA) is nonexisting, and information using the translabyrinthine approach (TLA) is scarce. We aimed to evaluate cochlear MRI fluid signal in patients with a unilateral VS, before and after surgery by the RLA or the TLA, that can have clinical importance for subsequent cochlear implantation feasibility. Study Design: Retrospective cohort study. Setting: University...

“Demineralized Bone Matrix and Resorbable Mesh Bilaminate Cranioplasty is Ineffective for Secondary Reconstruction of Large Pediatric Cranial Defects”
Summary: Replacement of the autologous bone flap after decompressive craniectomy can be complicated by significant osteolysis or infection with large defects over scarred dura. Demineralized bone matrix (DBM) is an alternative to autologous reconstruction, effective when reconstructing large defects using a resorbable mesh bilaminate technique in primary cranioplasty, but this technique has not been studied for revision cranioplasty and the setting of scarred dura. Retrospective review was performed...

Facial Recognition Neural Networks Confirm Success of Facial Feminization Surgery
Background: Male-to-female (MtF) transgender patients desire to be identified and treated as female, not only with partners but also in public and social settings. Facial Feminization Surgery (FFS) entails a combination of highly visible changes in facial features which may affect social “first impressions.” No study to date has evaluated the impact of FFS on how MtF patients are gender-typed. To study the effectiveness of FFS, we investigated preoperative/postoperative gender-typing using facial...

Bilobed Gracilis flap: A Novel Alternative for Pelvic and Perineal Reconstruction
Perineal reconstruction historically has been guided by the vertical rectus abdominus myocutaneous flap. In oncologic patients, due to prior surgical intervention this donor site is often unavailable, the pelvis has been radiated, and defects can be deep or irregularly contoured. Using plastic surgery principles of perforators, geometrically defined local tissue rearrangement, and flap inset our senior surgeons have developed a modification of the gracilis flap to include a second soft tissue arm...

Free Fibula Flap for Restoration of Spinal Stability After Oncologic Vertebrectomy is Predictive of Bony Union
Background: Nonvascular bone grafts >4 cm have a 50% non- union rate in spine surgery. Vascular bone flaps are one potential solution, however their utility in spine surgery has not been fully elucidated. We hypothesized that the addition of a free fibula flap (FFF) after oncologic vertebrectomy would safely potentiate bone union. Methods: We performed a retrospective analysis, including all patients who received an oncologic vertebrectomy for a primary bone tumor at our institution during...

Transfusion-Free Cranial Vault Remodeling: A novel, multi-faceted approach
Background: Approximately 1/2000 babies are born with craniosynostosis and primary open repair is typically performed before one year of age. Historically the procedure has been associated with nearly 100% transfusion rates. To decrease the rates of transfusion, our center has developed a novel multi-modal blood conservation protocol. Methods: We performed our standard of care to children aged one year or younger undergoing primary repair of craniosynostosis between 2008 and 2014. In 2014...
